How she works

"You don’t need to understand the jargon. That’s my job."

Most people leave a blood test with a report full of red flags and no idea what to do next. Jaceme’s consultations work the other way around: she walks through every marker with you , in plain language, and leaves you with a plan — what to eat, what to adjust, and when to re-test.

Her clinical background spans the conditions Malaysian families actually face: diabetes, hypertension, kidney disease and cancer-recovery nutrition — supported by international certifications in obesity management and diabetes education.

Medical nutrition therapy

Diabetes, hypertension and metabolic optimisation — grounded in your actual blood work, not generic advice.

Renal & chronic disease nutrition

Chronic kidney disease and hemodialysis nutrition support, planned around your treatment.

Oncology nutrition support

Practical, gentle nutrition care for patients during and after cancer treatment.

Functional & integrative nutrition

Root-cause thinking that connects your labs, lifestyle and long-term goals.

Clinical specialisms

The conditions that need more than general advice.

Not all nutrition advice is the same. The conditions below need medical nutrition therapy rather than general healthy-eating tips — protein targets that move with kidney function, appetite and taste changes during chemotherapy, lipid patterns that respond to specific fats. Jaceme works in these areas alongside the medical team already treating you, never in place of it.

Her certifications map directly onto them: SCOPE certification from the World Obesity Federation for obesity management, International Diabetes Educator accreditation from the IDF for metabolic disease, and a Nutrition Physical Focused Findings certification for assessing muscle loss and nutritional status in consultation.

Oncology nutrition

Nutrition support through and after cancer treatment: protecting weight and muscle when appetite, taste and tolerance change, and meeting the higher protein needs treatment creates. Planned around your oncology team, never instead of it.

Kidney disease

Chronic kidney disease and dialysis nutrition, where protein, potassium, phosphate, sodium and fluid all have to be balanced against each other, then rebalanced as your stage or dialysis schedule changes.

Cardiovascular health

Cholesterol and blood pressure worked at the level of actual eating patterns: which fats replace which, sodium in the food you really buy, and the fibre and weight changes that move lipids fastest.

Metabolic disease

Type 2 diabetes, insulin resistance, fatty liver and the cluster that travels with them. As an IDF-accredited diabetes educator, Jaceme plans around real meals, medication timing and glucose patterns rather than a fixed diet sheet.

Severe obesity

SCOPE-certified obesity management for people whose weight has not responded to general advice, treating obesity as a medical condition with staged, realistic targets rather than a willpower problem.

Why she co-founded HDC

“A blood test should end in a conversation , not a PDF.”

Too many people take a health screening, receive a report full of numbers — and never hear the “so what”. Jaceme co-founded HDC Medical to change that: a dietitian-led clinic where every screening comes with a one-to-one review, in plain language, in your language.

That principle shapes everything at HDC — from which blood test packages are offered, to the customized supplement formulas, to the follow-up that makes sure the plan actually happens.
